Training and employer review plan
This is a planned learning agenda. Record actual instruction and activities; breaks and idle time do not count. Extend training if a learner needs more practice. The short study guides alone do not establish five hours.

Employer review before sign-off
- Provide the current handbook and applicable employment terms. Record the policy version and questions answered.
- Confirm hire, affiliation and registration details; complete required training before client presence.
- Document actual date, title, topics, duration, provider, instructor where applicable, location or website, and caregiver identification in the training record.
- Review learning activities and assessments. Repeat instruction where needed; the website’s passing score is a course standard.
- Review supporting evidence in the application administration area. Retain training verification in the personnel file.
Do not upload client names or identifiable client information in learning activities.
