Pay, paid-sick-leave & employer notice

Official forms / guidance →Disability & paid-family-leave notices
California hiring notices — applicable coverage
Record delivery of DE 2515 and DE 2511 for covered new hires. This is evidence of delivery, not a requirement for the caregiver to apply for benefits.
Official forms / guidance →Harassment prevention policy / training
Employer-size and role requirements
Administration verifies the applicable policy notices and, for employers with 5 or more employees, required supervisory or nonsupervisory training. Track its separate actual renewal date.
Official forms / guidance →California workplace rights notice
California employer notice
Record delivery of the current required workplace-rights notice, including the annual requirement beginning in 2026. Administration reviews current timing and applicability.
Official forms / guidance →Signed employer policies / employment paperwork
Employer
Upload the signed policies requested by Excellent Caretakers. Tax, banking and I-9 identity documents are handled separately by HR, not in this portal.
How training is recorded
Quiz completion records learning progress. A five-hour training certificate requires reviewed evidence of two hours of employer orientation and three hours of safety training. The two-hour orientation must be completed for each employer. Annual training is a separate five-hour requirement based on your hire anniversary; it does not renew simply by reprinting your orientation certificate.
Annual topics include client rights and safety, daily living, abuse prevention and reporting, personal hygiene, and safe transportation when applicable. Dementia care becomes an explicit annual topic January 1, 2027.
Medical documents, criminal-history statements and identity documents are never shared with the agency directory. Tax, banking and work-authorization paperwork is handled separately by HR.
